Job summary

Civita Care Center at Long Ridge in Stamford, CT is hiring a Physical Therapist (PT) for a full-time position. You will assess residents, design and implement therapy plans, and collaborate with an interdisciplinary team to promote mobility and independence.

Qualifications

• Active Connecticut Physical Therapist license required.
• Graduate of an accredited Physical Therapy program.
• Strong clinical, communication, and organizational skills.
• Skilled nursing or rehabilitation experience preferred, but not required.
• New graduates are welcome to apply.

Responsibilities

• Evaluate residents and develop individualized physical therapy plans of care.
• Provide skilled treatment addressing mobility, gait, balance, strength, transfers, and endurance.
• Establish functional rehabilitation goals and track resident progress.
• Adjust treatment approaches based on resident needs and clinical response.
• Educate residents and caregivers on exercises, mobility strategies, and safety techniques.
• Maintain accurate and timely clinical documentation.
• Partner with PTAs, OT, SLP, nursing, and other team members.
