Job summary

Post Rehabilitation Services is seeking a Physical Therapist (PT) to join the rehabilitation team at Civita Care Center at Salmon Brook in Glastonbury, CT. This full-time opportunity supports residents in a skilled nursing and post-acute setting, focusing on restoring mobility and independence.

The role involves evaluating patients, developing plans of care, and coordinating with PTAs, OT, SLP, nursing, and the broader care team. New graduates are welcome to apply, and a CT license is required.

Qualifications

• Active Connecticut Physical Therapist license required.
• Graduate of an accredited Physical Therapy program.

Responsibilities

• Perform physical therapy evaluations and develop individualized plans of care.
• Provide skilled treatment focused on strength, balance, gait, transfers, endurance, and functional mobility.
• Establish resident-centered goals and monitor progress throughout the rehabilitation process.
• Modify treatment approaches based on clinical findings and resident response.
• Educate residents and caregivers on exercises, mobility techniques, and safety strategies.
• Complete accurate and timely clinical documentation.
• Collaborate with PTAs, OT, SLP, nursing, and other members of the interdisciplinary care team.
