Job summary

Evolve Therapy Services is seeking a Physical Therapist to provide in-house skilled nursing facility services on a PRN basis in Oklahoma. You will evaluate residents, develop treatment plans, and deliver therapy aligned with standards and CMS expectations through NetHealth EMR.

Responsibilities include screening residents, documenting care, and collaborating with an interdisciplinary team to ensure quality resident outcomes.

Qualifications

• Minimum of B.S. in Physical Therapy from an accredited program.
• Active Physical Therapy license in applicable states.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services to residents per established standards.
• Adhere to documentation and billing procedures.
• Evaluate residents to guide treatment planning.
• Develop and implement treatment plans with short and long-term goals.
• Communicate findings to residents, families, and the care team.
• Complete CMS-compliant documentation in NetHealth EMR.
