Job summary

Evolve Therapy Services, LLC is hiring a Physical Therapist for an in-house Skilled Nursing Facility in Tulsa, OK. The role is PRN and focuses on planning, directing, and delivering physical therapy services to residents in accordance with applicable standards and CMS expectations.

Responsibilities include evaluating residents, developing treatment plans with clear goals, and coordinating with families and the interdisciplinary team.

Qualifications

• Active Physical Therapy license in state(s) of practice.
• Bachelor of Science in Physical Therapy from an accredited program.
• Ongoing continuing education to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents per standards and productivity expectations.
• Document and bill therapy services per procedures.
• Develop treatment plans with short and long-term goals.
• Communicate findings to residents, families, and interdisciplinary team.
• Complete CMS compliant documentation in NetHealth EMR.
