Job summary

Evolve Therapy Services is seeking a licensed Physical Therapist to join our in-house skilled nursing facility team in Oklahoma. You will evaluate residents, design and implement individualized treatment plans, and provide evidence-based therapy to enhance resident outcomes.

This PRN position requires adherence to CMS-compliant documentation in NetHealth EMR, collaboration with the interdisciplinary team, and ongoing professional development to maintain licensure.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure.
• Assume responsibility for ongoing continuing education and professional development.

Responsibilities

• Provide physical therapy services to residents per standards of practice and department procedures.
• Adhere to documentation and billing procedures for physical therapy.
• Demonstrate sound clinical judgment in evaluation, planning, and follow-up of therapy programs.
• Conduct screenings of residents to determine need for intervention/treatment.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short and long-term goals and methods to achieve them.
• Communicate evaluation findings and recommendations to residents, families, and interdisciplinary team.
