Job summary

Evolve Therapy Services, LLC is seeking a licensed Physical Therapist for an in-house Skilled Nursing Facility in Tulsa, OK. The PT will provide comprehensive therapy services, evaluate residents, develop and implement treatment plans, and document according to CMS and NetHealth EMR standards.

Responsibilities include progress monitoring, communication with families and the interdisciplinary team, and adherence to licensure requirements.

Qualifications

• Bachelor of Science in Physical Therapy required.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents per standards.
• Adhere to documentation and billing procedures.
• Evaluate residents and develop treatment plans with goals.
• Communicate evaluation findings to residents, families and interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR.
• Maintain compliance with regulations to assure quality resident care.
