Job summary

Evolve Therapy Services, LLC is seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility in Tulsa, OK. This PRN role focuses on delivering quality PT services and collaborating with the interdisciplinary team to meet resident goals.

Applicants must hold a B.S. in Physical Therapy, a valid state license, and commit to ongoing professional development and compliant documentation per CMS standards.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.
• Assume responsibility for ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and department procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation and follow-up of resident therapy programs.
• Conduct screenings of residents to determine need for intervention/treatment.
• Develop treatment plans with short and long-term goals and methods to achieve identified goals.
• Interpret and communicate evaluation findings and treatment recommendations to residents and families.
