Evolve Therapy Services in Tulsa, OK is seeking a Physical Therapist for an in-house skilled nursing facility on a PRN basis. You will assess residents, plan and deliver therapy, and monitor progress in coordination with the care team.

The role involves evaluating residents, establishing short- and long-term goals, and documenting outcomes. CMS documentation is completed in NetHealth EMR, with opportunities for growth based on performance.

Formación

• Minimum of Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.
• Assume responsibility for ongoing continuing education and professional development to maintain current licensure and certification.

Responsabilidades

• Provide physical therapy services to residents in accordance with established standards of practice and department procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of resident therapy programs.
• Conduct screening of residents at regular intervals to determine need for intervention/treatment.
• Conduct timely screening of residents referred to physical therapy to determine need for intervention/treatment.
• Evaluate residents to obtain data for treatment planning and implementation.
• Conduct specialized evaluations as indicated.
• Develop treatment plans with short and long-term goals and achieving methods.
• Interpret and communicate evaluation findings, treatment plan and recommendations to residents, families, and interdisciplinary team.
• Complete compliant and medically necessary CMS documentation in NetHealth EMR systems.
