Qualifications

• BS or higher in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure/certification.

Responsibilities

• Provide PT services to residents per standards of practice and department procedures.
• Adhere to documentation and billing procedures for therapy services.
• Evaluate residents, develop and implement treatment plans with short and long-term goals.
• Communicate findings and recommendations to residents, families, and interdisciplinary team members.
• Complete CMS-compliant documentation in NetHealth EMR system.
