Job summary

LifeSpan Therapy is seeking a dedicated Physical Therapist (PT) in Paoli, Indiana. This role focuses on assessing residents, designing personalized treatment plans, and collaborating with a multidisciplinary team to restore mobility and independence.

Responsibilities include monitoring progress, educating residents and families, and occasionally traveling to other sites for coverage. Benefits include a $10,000 sign-on and opportunities for leadership growth within LifeSpan.

Qualifications

• Must be a licensed Physical Therapist (PT) and graduate of an ACOTE-accredited program.
• Current state licensure to practice physical therapy.
• Must have a valid Indiana Operator's license.
• Demonstrated experience in resident care, including assessment and treatment planning.
• Basic computer literacy for documentation and communication purposes.

Responsibilities

• Ensure PT services promote resident mobility, pain reduction and functional improvement.
• Assess residents' PT needs and develop individualized treatment plans.
• Coordinate admissions/discharges with care team for continuity of care.
• Develop and implement personalized treatment plans based on resident needs.
• Supervise or delegate tasks to PTAs and communicate plans to staff for team approach.
• Monitor progress and document outcomes in line with regulations.
