Job summary

LifeSpan Therapy in New Albany, Indiana, is seeking a Physical Therapist (PT) for a PRN position. You will assess residents, develop personalized treatment plans, and collaborate with multidisciplinary teams to promote mobility and independence after injuries or illnesses.

Qualifications

• Must be a licensed Physical Therapist (PT) who is a graduate of an ACOTE accredited school.
• Current state licensure to practice physical therapy.
• Must have a valid Indiana Operator's license.
• Demonstrated experience in resident care, including assessment and treatment planning.

Responsibilities

• Ensure PT services achieve optimal resident outcomes and comply with MDS procedures.
• Assess residents for PT needs at admission or as indicated.
• Coordinate PT admissions/discharges with the care team.
• Develop and implement individualized treatment plans based on resident needs.
• Assign PTAs when appropriate and communicate the plan to the team.
• Monitor progress and document outcomes in line with regulations.
• Educate residents and families on injury prevention and health maintenance.
• Utilize basic computer skills to manage records and schedule appointments.
• Occasionally travel to other locations to provide PT coverage.
