Job summary

LifeSpan Therapy in Indiana is seeking a Physical Therapist (PT) to help residents regain mobility and independence after injury, surgery, or illness. You will assess residents, create personalized plans, and work with families and a multidisciplinary team to achieve optimal outcomes.

Qualifications

• Must be licensed as a Physical Therapist (PT) from an ACOTE-accredited program.
• Current Indiana licensure to practice physical therapy.
• Must have a valid Indiana Operator's license.
• Experience in resident care, including assessment and treatment planning.
• Basic computer literacy for documentation and communication.

Responsibilities

• Ensure PT services foster optimal resident outcomes per MDS procedures.
• Assess residents for PT needs at admission or as indicated.
• Coordinate PT admission/discharge with the care team.
• Develop and implement individualized PT treatment plans.
• Supervise PTAs when appropriate and communicate plans to staff.
• Monitor progress and document outcomes per regulations.
• Educate residents and families on injury prevention and therapy techniques.
• Manage resident records and scheduling using basic computer skills.
• Occasionally travel to provide PT coverage at other locations.
