Job summary

LifeSpan Physical Therapy Services LLC seeks a dedicated Physical Therapist (PT) to support residents in our Anderson, Indiana facility. You will assess residents, develop personalized plans, and implement interventions to improve mobility and reduce pain after injury, surgery, or illness.

Qualifications

• Must be a licensed Physical Therapist (PT) who is a graduate of an ACOTE accredited school.
• Current state licensure to practice physical therapy.
• Must have a valid Indiana Operator's license.
• Demonstrated experience in resident care, including assessment and treatment planning.
• Basic computer literacy for documentation and communication purposes.

Responsibilities

• Assures that Physical Therapy Services facilitate optimal resident outcomes and complies with MDS procedures.
• Assesses facility residents for PT needs at admission or when indicated to provide appropriate services.
• Coordinates admission/discharge to Physical Therapy with the Interdisciplinary Team and nursing staff.
• Develop and implement individualized treatment plans based on resident needs and clinical best practices.
• Performs therapy or assigns resident to a PTA and communicates information about the treatment plan to nursing staff.
• Monitor resident progress and document treatment outcomes per healthcare regulations.
• Educate residents and families on injury prevention and exercises.
• Travel to other locations as needed for PT coverage.
