Job summary

LifeSpan is seeking a Physical Therapist (PT) to join a family-owned, multi-location Indiana team. You will assess residents, design personalized treatment plans, and collaborate with families and a multidisciplinary team to promote mobility and independence.

Qualifications

• Must be a licensed Physical Therapist (PT) from an ACOTE accredited school.
• Current state licensure to practice physical therapy.
• Must have a valid Indiana Operator's license.
• Demonstrated experience in resident care, including assessment and treatment planning.
• Basic computer literacy for documentation and communication purposes.

Responsibilities

• Assures that Physical Therapy Services facilitate optimal resident outcomes and complies with the MDS procedures.
• Assesses facility residents for PT needs at admission or when indicated to provide appropriate services.
• Coordinates admission/discharge to Physical Therapy with the Interdisciplinary Team and staff nurses.
• Develop and implement individualized treatment plans based on resident needs and clinical best practices.
• Performs therapy or assigns resident to a PTA and communicates treatment plans to nursing and other personnel.
• Monitor resident progress and document treatment outcomes accurately in compliance with regulations.
• Educate residents and families on injury prevention, health maintenance, and therapy techniques.
• Utilize basic computer skills to manage resident records and schedule appointments.
• Occasionally travels to other locations to provide PT coverage when needed.
