Job summary

Life Care Centers of America is seeking a Physical Therapist to provide direct rehab care to patients referred by physicians. You will work within a skilled nursing facility alongside an interdisciplinary team to deliver patient-centered care in compliance with all regulations and Life Care standards.

The position requires graduation from an accredited PT program and active licensure in the state. New grads are welcome, with CPR certification and ongoing professional development encouraged.

Qualifications

• Graduate of an accredited physical therapy program (BSPT/MSPT/DPT).
• Current state license to practice physical therapy.
• One year experience preferred; new grads welcome.

Responsibilities

• Follow and implement established physical therapy treatment plans.
• Establish and modify realistic, measurable goals for patients.
• Oversee and evaluate care by PTAs, rehab aides, and students.
• Chart patient progress accurately and timely.
• Utilize therapy software and documentation systems correctly.
• Provide excellent customer service and maintain a positive attitude.
• Assist in patient evacuation when needed.
• Maintain professional relationships with colleagues and patients.
• Adhere to confidentiality and Life Care policies and conduct.
