West Roxbury Health & Rehabilitation Center is seeking a compassionate Physical Therapist Assistant to join our rehabilitation team. You will provide skilled therapy under the direction of a licensed Physical Therapist while building meaningful relationships with residents and helping them achieve their individualized goals.

Qualifications

  • Graduation from an accredited Physical Therapist Assistant program required.
  • Current state license or eligibility for licensure as a Physical Therapist Assistant required.
  • Current CPR/BLS certification required.
  • Long-term care or skilled nursing experience preferred.
  • Experience working with geriatric and subacute rehabilitation populations preferred.
  • Strong interpersonal and communication skills.
  • Ability to work collaboratively with residents, families, and interdisciplinary healthcare teams.
  • Compassionate, dependable, and resident-focused approach to care.
  • English language proficiency required.

Responsibilities

  • Assist the Physical Therapist in planning, developing, and implementing individualized treatment programs for residents.
  • Provide skilled physical therapy treatments according to the established plan of care and within the scope of practice.
  • Assist residents in improving mobility, strength, balance, coordination, and functional abilities.
  • Monitor resident progress and document observations, treatments, and outcomes accurately and timely.
  • Maintain clinical documentation, treatment records, and departmental reports according to regulatory requirements.
  • Collaborate with interdisciplinary team members regarding resident progress, care planning, and discharge recommendations.
  • Educate residents, families, and caregivers on therapy techniques and recommendations to support continued progress.
  • Manage assigned caseload by coordinating individual and group treatment sessions.
  • Follow safety, infection control, and regulatory standards.
  • Perform other duties as assigned.

  • Follow all safety, infection control, and regulatory standards.
  • Perform other duties as assigned.
