Job summary

TEEMA Solutions Group is seeking a dedicated Physical Therapist to provide patient-centered rehabilitation in a healthcare setting. You will evaluate patients, develop individualized plans, and apply evidence-based interventions to restore mobility and reduce pain.

You will collaborate with physicians and interdisciplinary teams, maintain documentation, educate patients and families, and ensure safety and quality standards across care settings.

Qualifications

• Active PT license in the practice state.
• DPT degree or equivalent per state regulations.
• BLS/CPR certification as required by facility policy.
• Experience in acute care, outpatient, inpatient rehab, SNF, home health, or specialty settings.
• Strong assessment, communication, documentation, and patient education skills.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide interventions to improve mobility, strength, balance, and pain management.
• Monitor progress and modify plans; document evaluations and outcomes.
• Educate patients and families on treatment plans and home programs.
• Collaborate with physicians and interdisciplinary teams to coordinate care.
• Maintain accurate clinical documentation per policy.
• Ensure patient safety, infection control, and QA standards.
• Treat patients across assigned settings and age groups.
• Perform other duties as assigned.
