Amedisys is seeking a compassionate Physical Therapist to evaluate and treat patients in their homes in Bradley and Polk Counties. You will design individualized treatment plans to restore movement, strength and independence while coordinating discharge planning with care teams.

The role runs Fri–Sun, 3 shifts per week with 10-hour days, and offers competitive pay and robust benefits. You will join a patient‑focused home health team dedicated to transforming lives at home.

Qualifications

• Current license to practice physical therapy in the assigned state.
• Graduate from a CAPTE-accredited physical therapy program.
• One year of home health PT experience recommended; if <1 year, mentoring may be required.
• Current CPR certification.

Responsabilités

• Perform interventions, reexaminations (every 30 days at minimum), modify plans as needed and discharge planning.
• Other duties as assigned.

Responsibilities

• Responsible for helping patients maintain, restore and improve movement, activity and function.

• Provides interventions, conducts reexaminations (every 30 days, at minimum), modifies interventions as necessary, and performs discharge planning.
• Other duties as assigned.

Qualifications

• Current license to practice physical therapy specific to the state you're assigned to work in.

• Graduate from an accredited physical therapy educational program as set by the Commission on Accreditation of Physical Therapy Education (CAPTE).
• One year related physical therapy experience for PTs entering home health care to ensure that knowledge and skills are sufficient to safely provide physical therapy services to patients.
• If less than one year of experience, approval from regional clinical leaders is required as well as appropriate mentoring and/or participating in a residency program.
• Current CPR certification.
