Job summary

Amedisys, a leading home health and hospice provider, is seeking a Physical Therapist in Tennessee to support patients at home by evaluating needs and developing interventions to improve movement and function. You will monitor progress and reexaminations, and plan discharges as appropriate.

State licensure, CAPTE accreditation, CPR certification, and at least one year of home health experience are preferred; mentoring can be provided for less experience.

Qualifications

• Current license to practice physical therapy in the assigned state.
• Graduate from a CAPTE-accredited physical therapy program.
• One year related physical therapy experience for home health care; if <1 year, regional leadership approval and mentoring.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions, conduct reexaminations every 30 days minimum, modify interventions as needed, and perform discharge planning.
• Other duties as assigned.

• Other duties as assigned.

Qualifications

• Current license to practice physical therapy specific to the state you're assigned to work in.
• Graduate from an accredited physical therapy educational program as set by the Commission on Accreditation of Physical Therapy Education (CAPTE).
• One year related physical therapy experience for PTs entering home health care to ensure that knowledge and skills are sufficient to safely provide physical therapy services to patients.
• If less than one year of experience, approval from regional clinical leaders is required as well as appropriate mentoring and/or participating in a residency program.
• Current CPR certification.
