Job summary

AMEDISYS HOME HEALTH is seeking a compassionate Physical Therapist to deliver in-home care in Bradley and Polk Counties, Tennessee. You will help patients regain strength, mobility and independence through personalized interventions, ongoing assessments, and discharge planning in a home setting.

Expect a Fri-Sat-Sun schedule with 3 ten-hour shifts. Compensation ranges from 58.00 to 60.00 per hour, with benefits including healthcare options, PTO, wellness rewards, and retirement plans.

Qualifications

• Current license to practice physical therapy in the assigned state.
• Graduate from an accredited physical therapy program per CAPTE.
• One year of related PT experience for home health or mentoring/ residency if less than one year.
• CPR certification current.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions, conduct 30-day reexaminations, modify plans as needed, and perform discharge planning.
• Other duties as assigned.

Responsibilities

• Responsible for helping patients maintain, restore and improve movement, activity and function.
• Provides interventions, conducts reexaminations (every 30 days, at minimum), modifies interventions as necessary, and performs discharge planning.
• Other duties as assigned.

Qualifications

• Current license to practice physical therapy specific to the state you're assigned to work in.
• Graduate from an accredited physical therapy educational program as set by the Commission on Accreditation of Physical Therapy Education (CAPTE).
• One year related physical therapy experience for PTs entering home health care to ensure that knowledge and skills are sufficient to safely provide physical therapy services to patients.
• If less than one year of experience, approval from regional clinical leaders is required as well as appropriate mentoring and/or participating in a residency program.
• Current CPR certification.
