Job summary

AMEDISYS HOME HEALTH is seeking a compassionate Physical Therapist to deliver skilled home care in Kingsport, TN. You will assess, treat, and discharge patients in their homes, focusing on restoring movement, mobility, and independence.

This role emphasizes ongoing assessments, mid-cycle reexaminations every 30 days, and collaborative planning with families and care teams to ensure smooth transitions and durable outcomes.

Qualifications

• Must hold a current Physical Therapy license for the state of assignment (Tennessee).
• Graduate from CAPTE-accredited PT program (DPT preferred).
• One year of home health PT experience recommended; mentoring possible for less experienced cases.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Perform reexaminations at least every 30 days and modify interventions as needed.
• Complete discharge planning and other duties as assigned.

Responsibilities

• Responsible for helping patients maintain, restore and improve movement, activity and function.
• Provides interventions, conducts reexaminations (every 30 days, at minimum), modifies interventions as necessary, and performs discharge planning.
• Other duties as assigned.

Qualifications

• Current license to practice physical therapy specific to the state you’re assigned to work in.
• Graduate from an accredited physical therapy educational program as set by the Commission on Accreditation of Physical Therapy Education (CAPTE).
• One year related physical therapy experience for PTs entering home health care to ensure that knowledge and skills are sufficient to safely provide physical therapy services to patients.
• If less than one year of experience, approval from regional clinical leaders is required as well as appropriate mentoring and/or participating in a residency program.
• Current CPR certification.
