Job summary

Amedisys in Kingsport, TN is seeking a licensed Physical Therapist to join our home health team on weekends (Fr, Sat, Sun). You will help patients regain movement, strength, and independence in their homes, with individualized plans and regular reexaminations.

Responsibilities include delivering targeted interventions, adjusting care plans, and coordinating discharge planning. A commitment to compassionate care and ability to work autonomously in the home setting are essential.

Qualifications

• Current license to practice physical therapy in the assigned state.
• Graduate from CAPTE-accredited physical therapy program.
• One year of related home health PT experience; or approval/mentoring if less than one year.
• Current CPR certification.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions, conduct reexaminations (every 30 days) and modify interventions as necessary; perform discharge planning.
• Other duties as assigned.

Qualifications

• Current license to practice physical therapy specific to the state you're assigned to work in.
• Graduate from an accredited physical therapy educational program as set by the Commission on Accreditation of Physical Therapy Education (CAPTE).
• One year related physical therapy experience for PTs entering home health care to ensure that knowledge and skills are sufficient to safely provide physical therapy services to patients.
• If less than one year of experience, approval from regional clinical leaders is required as well as appropriate mentoring and/or participating in a residency program.
• Current CPR certification.
