Job summary

Sigma Health Rehab is seeking a weekend Physical Therapist (PT) to join our skilled team in a Skilled Nursing Facility in Bath, New York. You will evaluate residents and create rehabilitation plans, provide relief through exercises and hands-on therapy, and supervise PT assistants.

Qualifications

• Must graduate from an accredited Physical Therapy program.
• Must hold an active New York State PT license.

Responsibilities

• Evaluate residents’ conditions and develop appropriate physical rehabilitation plans.
• Relieve residents’ pain using exercises, hands-on therapy & stretching.
• Help residents through their tasks and monitor their performance.
• Supervise the activities of physical therapy assistants.
• Maintain resident confidence and protect facility by keeping information confidential.
• Complies with federal, state, and local legal and professional requirements.
• Maintains a cooperative relationship among healthcare team members and contributes to team effort.

Education

Graduate of an accredited Physical Therapy Program

Current New York State PT License
