Job summary

Sigma Health Rehab in New York—Williamsbridge Center for Rehabilitation and Nursing—seeks a licensed Physical Therapist (PT) for a full-time role in a skilled nursing facility. The PT will evaluate residents, design rehabilitation plans, and guide therapy teams in a collaborative, patient-centered environment.

The role emphasizes excellent patient care, adherence to standards, and ongoing professional development through CEU reimbursement and mentorship. NY licensure is required or in progress.

Qualifications

• Graduate of an accredited Physical Therapy Program.
• Current New York State PT License.
• Experience in a skilled nursing or geriatric care setting preferred; new graduates with relevant fieldwork encouraged to apply.

Responsibilities

• Evaluate residents' conditions and develop appropriate physical rehabilitation plans.
• Relieve residents' pain using exercises, hands-on therapy and stretching.
• Help residents through their tasks and monitor their performance.
• Supervise the activities of physical therapy assistants.
• Maintain resident confidentiality and protect facility information.
• Comply with federal, state, and local legal and professional requirements.
• Maintain a cooperative relationship among healthcare team members.
