Job summary

Sigma Health Rehab is seeking a per diem Physical Therapy Assistant (PTA) to work in a Skilled Nursing Facility in Bath, New York. The PTA will support therapy goals under supervision and collaborate with the PT to implement the rehabilitation plan while assisting residents with daily activities.

Ideal candidates are graduates of an accredited PTA program, hold an active New York State PTA license, and value mentorship, flexible scheduling, and ongoing clinical support provided by our team.

Qualifications

• Must be a graduate of an accredited PTA program.
• Current New York State PTA License is required.

Responsibilities

• Select therapy activities that follow the plan of care.
• Assist residents in daily activities.
• Monitor residents' performance and document progress.
• Teach residents to use equipment to aid daily function.
• Collaborate with PT to establish rehabilitation plan.
• Maintain cooperative relationships within the healthcare team.

Duties of an exceptional therapist include:

• Select therapy activities that follow the plan of care set out by the supervising occupational therapist
• Assist residents in carrying out daily activities.
• Monitor residents' performance in therapy activities, provide encouragement and document on residents as needed.
• Teach residents how to use special equipment to assist with daily function.
• collaborate with the PT to establish a rehabilitation plan.
• Maintains a cooperative relationship among healthcare team members & contributes to team effort
