Responsibilities

What you will do: Responsibilities include but are not limited to:

• Perform evaluations and develop treatment plans
• Re-assess effectiveness of treatment plans, attend staffing meetings and family conferences
• Perform discharge evaluations, make recommendations for home and community safety, and provide referrals and education resources
• Communicate patient’s needs and progress to the treatment team, physicians, clients, and family members
• Collaborate with other disciplines to ensure coordinated care and improve patient outcomes

Qualifications

What you will need:

• Bachelor’s degree in Physical Therapy from an accredited program
• Current, unrestricted Physical Therapist license in the practicing state
• Current BLS or CPR certification as per state regulations
• Preferred: at least one year of work experience as a physical therapist
• Knowledge of rehabilitation techniques and therapeutic interventions for adults
• Effective verbal and written communication skills
• Ability to lift up to 50 pounds; may involve lifting up to 100 pounds with proper safety procedures
• Physical requirements include lifting, moving, standing, walking, pushing, pulling, bending, reaching, kneeling, and positioning
• A health screen or exam may be required prior to and periodically during employment
