Overview

This position offers a $2,500 Sign-on Bonus.

Position Details: Licensed Physical Therapist (PT) supporting our innovative care coordination program, allowing practice at the top of your license, with opportunities to expand into care coordination duties. Responsibilities

• Provide physical therapy and wellness services to older adults in senior living communities.
• Conduct assessments, develop treatment plans, and perform therapeutic interventions as part of a multidisciplinary team.
• Support care coordination by conducting wellness check-ins, communicating with providers and caregivers, and assisting with telehealth visits.
• Adhere to professional standards and company policies.
• Contribute to program development, quality improvement, and problem-solving initiatives.
• Enhance patient well-being and satisfaction.

Schedule

Flexible scheduling within typical clinic hours (8 a.m. to 5 p.m., Monday to Friday).
