Overview

This position offers a $2,500 Sign-on Bonus.

Position Details: Licensed Physical Therapist (PT) supporting our innovative care coordination program, allowing practice at the top of your license, with opportunities to expand into care coordination duties. Enjoy flexible scheduling, excellent benefits, and make a significant impact on seniors' lives.

Responsibilities
  • Provide physical therapy and wellness services to older adults in senior living communities.
  • Conduct assessments, develop treatment plans, and perform therapeutic interventions as part of a multidisciplinary team.
  • Support care coordination by conducting wellness check-ins, communicating with providers and caregivers, and assisting with telehealth visits.
  • Adhere to professional standards and company policies.
  • Contribute to program development, quality improvement, and problem-solving initiatives.
  • Enhance patient well-being and satisfaction.
Schedule

Flexible scheduling within typical clinic hours (8 a.m. to 5 p.m., Monday to Friday).
