Job summary

Outcome HC is seeking a PRN Physical Therapist to join our Stroudsburg, PA skilled nursing facility. You will evaluate and treat residents to improve mobility, balance, gait, and functional independence in subacute and long-term care settings.

Collaborate with physicians, nurses, PTAs, and other therapists; document progress in the EMR; participate in care plan meetings and family conferences; supervise PTAs as needed. PA license required or eligibility.

Qualifications

• Active Physical Therapist license in Pennsylvania or eligible to obtain.
• New grads welcome with mentorship and training provided.
• Experience in SNF, long-term care, or geriatrics is a plus.

Responsibilities

• Evaluate and treat residents in subacute and long-term care for orthopedic, neurological, and post-surgical conditions.
• Develop personalized treatment plans to improve strength, balance, gait, and functional mobility.
• Collaborate with physicians, nurses, PTAs, and speech therapists to coordinate care.
• Document patient progress accurately in the EMR in a timely manner.
• Participate in care plan meetings and family conferences.
• Provide compassionate, patient-centered care in a team environment.
• Supervise PTAs as needed and report to the Director of Rehabilitation.

What You’ll Do

• Evaluate and treat residents in subacute and long term care setting with a variety of orthopedic, neurological, and post-surgical conditions
• Develop personalized treatment plans to improve strength, balance, gait, and functional mobility
• Collaborate closely with physicians, nurses, occupational therapists, and speech therapists
• Document patient progress accurately and in a timely manner using our electronic medical records system
• Participate in care plan meetings and family conferences
• Provide compassionate, patient-centered care in a supportive team environment
• Supervise Physical Therapist Assistants (PTAs) as needed
• Report directly to the Director of Rehabilitation
