Uniontown Nursing and Rehab is seeking a compassionate and motivated PRN Physical Therapist (PT) to join our rehabilitation team in Uniontown, Pennsylvania.
This is a great opportunity for a Physical Therapist looking for flexible PRN hours while making a meaningful difference in the lives of residents in a Skilled Nursing and Long-Term Care setting.
As a PRN Physical Therapist, you will:
Evaluate residents and develop individualized plans of care
Provide skilled physical therapy services focused on mobility, strength, balance, safety, and functional independence
Establish measurable goals and monitor resident progress
Complete evaluations, progress reports, reassessments, and discharge documentation
Collaborate with nursing, therapy, physicians, and other members of the interdisciplinary care team
Educate residents, families, and caregivers regarding treatment plans and safety
Maintain accurate and timely clinical documentation using Net Health and PointClickCare (PCC)
Provide coverage based on facility and resident needs
Current Physical Therapist (PT) license in Pennsylvania or eligibility for licensure
Graduate of an accredited Physical Therapy program
Strong communication and clinical decision-making skills
Compassionate and resident-focused approach to care
Ability to work effectively as part of an interdisciplinary team
Skilled Nursing/LTC experience is a plus
