Qualifications

• Skilled Nursing/LTC experience is a plus

Responsibilities

• Evaluate residents and develop individualized plans of care
• Provide skilled physical therapy services focused on mobility, strength, balance, safety, and functional independence
• Establish measurable goals and monitor resident progress
• Complete evaluations, progress reports, reassessments, and discharge documentation
• Collaborate with nursing, therapy, physicians, and other interdisciplinary team members
• Educate residents, families, and caregivers regarding treatment plans and safety
• Maintain accurate and timely clinical documentation using Net Health and PCC
• Provide coverage based on facility and resident needs

Uniontown Nursing and Rehab is seeking a compassionate and motivated PRN Physical Therapist (PT) to join our rehabilitation team in Uniontown, Pennsylvania.

This is a great opportunity for a Physical Therapist looking for flexible PRN hours while making a meaningful difference in the lives of residents in a Skilled Nursing and Long-Term Care setting.

What You'll Do

As a PRN Physical Therapist, you will:

• Evaluate residents and develop individualized plans of care
