Job summary

Patewood Post Acute is seeking a compassionate Physical Therapist (PT) to join our rehabilitation team on a PRN basis in Greenville, SC. You will evaluate and treat residents, develop individualized plans, and collaborate with an interdisciplinary team to maximize independence. Requirements include a degree in Physical Therapy and SC licensure.

  • Bachelor's degree in Physical Therapy required.
  • Master's or Doctorate degree in Physical Therapy preferred.
  • Experience in skilled nursing, rehabilitation, or long‑term care preferred.
  • Experience with Casamba and PointClickCare (PCC) preferred.
  • Strong clinical, communication, and documentation skills.

Responsibilities

  • Evaluate residents and develop individualized treatment plans based on physician orders and clinical needs.
  • Provide skilled physical therapy services to improve mobility, strength, balance, endurance, and functional independence.
  • Monitor resident progress and modify treatment plans as appropriate.
  • Communicate regularly with physicians, nursing staff, residents, families, and interdisciplinary team members regarding treatment goals and outcomes.
  • Complete accurate and timely documentation in accordance with facility, state, federal, and payer requirements.
  • Participate in care plan meetings, rehabilitation conferences, and discharge planning activities.
  • Educate residents, families, and caregivers on therapy goals, safety techniques, and home exercise programs.
  • Conduct in-service education and training for facility staff as needed.
  • Ensure compliance with all state practice acts, facility policies, and regulatory requirements.
  • Promote resident safety, independence, and quality of life.

Position Summary

We are seeking a compassionate and skilled Physical Therapist (PT) to join our rehabilitation team on a PRN basis. Key Responsibilities

  • Communicate regularly with physicians, nursing staff, residents, families, and interdisciplinary team members regarding treatment goals and outcomes

Supervisory Responsibilities
  • Supervise Physical Therapist Assistants (PTAs), therapy aides, and students in accordance with state regulations and facility policies.
Qualifications
  • Bachelor's degree in Physical Therapy required
  • Master's or Doctorate degree in Physical Therapy preferred
  • Experience in skilled nursing, rehabilitation, or long‑term care preferred
  • Experience with Casamba and PointClickCare (PCC) preferred
  • Strong clinical, communication, and documentation skills
Licenses & Certifications
  • Current Physical Therapist license in South Carolina and in good standing
  • Must maintain all required continuing education requirements
  • Must remain in good standing with the applicable State Board
