Qualifications

• Degree in Physical Therapy required; Bachelor’s minimum, Master’s/Doctorate preferred.
• Active, unencumbered Physical Therapist license in South Carolina (required).
• Continuing education and good standing with State Board required.
• Long-term care or rehab setting experience preferred; PCC/Casamba familiarity helps.

Responsibilities

• Perform patient evaluations and develop individualized treatment plans.
• Provide therapy to improve strength, mobility, balance, and function.
• Monitor progress and adjust treatment plans accordingly.
• Maintain accurate and timely clinical documentation.
• Communicate with physicians, residents, families and care team members.
• Participate in care planning and discharge planning.
• Conduct staff education and in-service training.

Position Summary
