Job summary

Fountain Inn Post Acute in Fountain Inn, SC is seeking a Physical Therapist (PT) for PRN scheduling to evaluate and treat residents, improving mobility and functional independence. You will collaborate with physicians, families, and the rehab team to provide high-quality, individualized care.

Applicants should hold a Physical Therapy degree and SC licensure, with prior long-term care or rehabilitation experience preferred.

Qualifications

• Degree in Physical Therapy (Bachelor's required; Master's or Doctorate preferred).
• Active, unencumbered Physical Therapist license in South Carolina (required).
• Must maintain continuing education and stay in good standing with the State Board.
• Experience in a long-term care or rehab setting preferred.
• Familiarity with PointClickCare (PCC) and Casamba preferred.
• Strong clinical, communication, and organizational skills.

Responsibilities

• Perform patient evaluations and develop individualized treatment plans.
• Provide therapy to improve strength, mobility, balance, and overall function.
• Monitor patient progress and adjust treatment plans as needed.
• Maintain accurate and timely clinical documentation.
• Communicate with physicians, residents, families, and care team members.
• Participate in care planning and discharge planning.
• Conduct staff education and in-service training.
