CommuniCare Rehab, a member of the CommuniCare Family of Companies, has a PRN opening for a Physical Therapist (PT) at Willow Tree Healthcare Center. Our therapists are In-House Team Members. Qualifications

• A Bachelor's, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program.
• Hold a valid or pending State Physical Therapy License.

Job Responsibilities

• Evaluate, review, and screen patients'/residents' medical charts, perform evaluations under physician order, and develop a treatment plan tailored to patient needs.
• Implement treatment plans through direct treatment and educate team members, including providing supervision for PTAs.
• Attend or lead interdisciplinary meetings to relay clinical findings and make recommendations, as well as conduct staff and family training and education.
