Job Opportunity: Physical Therapist (PT) at BAYADA Home Health Care

BAYADA Home Health Care is seeking a full-time Physical Therapist (PT) for home health visits, assessments, and admissions. This role services adult and geriatric clients across territories in Stokes and Surry counties.

As a home care Physical Therapist, you will be a vital part of a multidisciplinary team providing rehabilitative and skilled nursing care, enabling clients to stay at home with appropriate medical support.

Responsibilities:

1. Make home visits to clients in designated areas.
2. Perform diagnostic tests and measurements related to mobility, strength, balance, and safety.
3. Implement wound care interventions using evidence-based dressings and topical agents.
4. Develop and execute individualized care plans including manual therapy, gait training, and balance exercises.
5. Assess and revise care plans regularly; collaborate with nursing staff.
6. Educate clients and families on rehabilitative activities to promote health and independence.
7. Document all observations and interventions accurately using electronic medical records.

Qualifications:

1. Current North Carolina Physical Therapist (PT) license.
2. Graduation from an accredited Physical Therapy program.
3. Ability to work independently and manage time effectively.
4. Strong interpersonal, organizational, and problem-solving skills.
5. Proficiency with computers; experience with EMR preferred.
