Ameriwell Care seeks a licensed Physical Therapist (PT) to provide in-home physical therapy services in the Elizabeth, NJ area. The role offers flexible scheduling, steady referrals, and a supportive agency team to help you impact patients' daily lives.

You will assess patients, develop individualized treatment plans, guide home exercise programs, and document findings in a timely, compliant manner while working independently in patients' homes.

Qualifications

• Active New Jersey Physical Therapist license required.
• Strong communication and interpersonal skills.
• Reliable transportation to travel between patients' homes.
• Ability to work independently and professionally.
