Job summary

Ignite Rehab (DBA Metro Physical & Aquatic Therapy) seeks a per diem Home Care Physical Therapist to provide in-home care in Sussex County, NJ. The role offers 1099 and W-2 options, flexible scheduling, and autonomy to see patients seven days a week.

We provide mentorship, a supportive team, and a focus on patient care with a quick EMR system (no Oasis required). Travel to patients’ homes within the geographic territory is required.

Qualifications

• Licensed Physical Therapist in New Jersey or eligible.
• Graduate of an approved Physical Therapy program.

Responsibilities

• Accept and treat patients in their homes within the assigned area.
• Provide 1:1 treatment.
• Develop individualized treatment plans per physician orders.
• Deliver care aligned with professional standards and plan of care.
• Educate family members and caregivers on proper therapy techniques.
• Carry out therapy programs to maintain or improve physical function.
• Perform initial exam and document findings.
• Provide timely documentation for each visit.
• Adjust treatments based on patient progress.
• Willing to treat geriatric patients.

Responsibilities:

• Accept and treat patients within your designated geographic area
• Provide 1:1 treatment
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs
• Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care
• Educate family members, home health aides, and other caregivers on proper therapy techniques
• Plan, and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.
• Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
• Provide timely and accurate documentation for each visit
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit
• Must be willing to treat geriatric patients.

• Physical Therapist Program Graduate
• Licensed Physical Therapist in the state of New Jersey or eligible.
• PreferredCertifications: First Aid, AED, & CPR Certifications.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory
