Job summary

BrightStar Care of Westfield is seeking an experienced Physical Therapist (PT) to provide one-on-one home health care in Union County, NJ. This per-diem role has no minimum hours and offers a flexible schedule with visits you set.

The role pays $70–$85 per visit and includes travel pay between cases, weekly pay with direct deposit, and the support of a Joint Commission accredited agency. Master’s degree in PT and NJ license required.

Qualifications

• Master’s degree in Physical Therapy from an accredited program.
• Active NJ PT license + CPR/BLS certification.
• Minimum 1 year of PT experience (home health preferred).

Responsibilities

• Evaluate patients, develop personalized care plans, and provide skilled PT treatments.
• Collaborate with physicians and Nursing leadership to coordinate care.
• Teach patients and caregivers safe mobility and exercise techniques.
• Monitor progress, document outcomes, and support discharge planning.
