Job summary

Beyond Home Health Care in New Jersey seeks a licensed Physical Therapist to provide in-home and community-based PT services for adults with developmental disabilities. The PT will evaluate mobility, strength, balance, gait, and functional movement, delivering person-centered interventions to maximize independence and participation.

The role requires NJ PT licensure, CAPTE-accredited education, CPR/BLS, and the ability to travel between homes.

Qualifications

• Current New Jersey license as a Physical Therapist (PT).
• Graduate of a CAPTE-accredited physical therapy program (DPT or equivalent).
• Minimum of one year of clinical experience preferred; new graduates considered.
• Current CPR/BLS certification.
• Clean criminal background check and fingerprinting per NJ DDD requirements.
• Reliable transportation, valid NJ driver’s license, and current auto insurance.

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans aligned with ISP and goals.
• Deliver one-on-one therapy services in home and community settings.
• Document services per DDD standards, including notes and outcome measures, on time.
• Collaborate with Support Coordinators, families, and the interdisciplinary team to ensure continuity of care.
• Provide caregiver training and home programs to reinforce goals between sessions.
• Adhere to DDD policies, privacy standards, and required trainings.
• Maintain billing records consistent with DDD rates and authorizations.
