Job summary

Homeside Rehab is seeking a compassionate Physical Therapist to join our home health team in Irvington, NJ. You will deliver individualized, evidence-based therapy to patients in their homes, with schedule flexibility and a supportive team environment.

The role requires a NJ PT license and a minimum of 1 year in home health, outpatient, or SNF settings. Expect field visits with a flexible day/evening schedule and opportunities for professional growth.

Qualifications

• Active NJ Physical Therapy License (PT) is required.
• Basic computer proficiency for electronic documentation.
• Strong clinical assessment and interpersonal communication skills.
• Ability to work independently in a home health environment.
• Valid NJ driver's license and reliable transportation for home visits.
• Minimum 1 year of experience in Home Health, Outpatient, or SNF settings.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on clinical assessment and patient goals.
• Provide evidence-based PT interventions to improve motor skills and functional independence.
• Maintain accurate, timely documentation of patient care in accordance with regulatory requirements.

Job Summary:

Job Responsibilities:

• Evaluate patients and develop individualized treatment plans based on clinical assessment and patient goals
• Provide evidence-based PT interventions to improve motor skills, functional independence, and developmental outcomes
• Maintain accurate, timely documentation of all patient care in accordance with regulatory requirements
• Collaborate with the clinical team to ensure coordinated and effective treatment
• Communicate clearly with office staff, patients, and caregivers regarding progress and care plans

Job Qualifications:

• Active NJ Physical Therapy License (PTA) — required
• Basic computer proficiency for electronic documentation

Job Details:

• Salary: $65 - $80 per hour
• Schedule Flexibility: Choose your own hours, day and evening shifts available, Monday to Friday, with weekend availability as needed
