Job summary

BrightStar Care of Westfield & Summit is seeking an experienced Physical Therapist (PT) to provide one-on-one home health care in Union County, NJ. This per-diem role has no minimum hours and offers flexible scheduling, delivering meaningful patient impact with independence and support from a dedicated team.

You will evaluate patients, develop tailored care plans, and deliver skilled PT treatments in patients' homes, collaborating with physicians and the Director of Nursing to optimize outcomes

Qualifications

• Master’s degree in Physical Therapy from an accredited program.
• Active NJ PT license + CPR/BLS certification.
• Minimum 1 year of PT experience (home health preferred).

Responsibilities

• Evaluate patients, develop personalized care plans, and provide skilled PT treatments.
• Collaborate with physicians & the Director of Nursing.
• Teach patients, families, and caregivers safe mobility & exercise techniques.
• Monitor progress, document outcomes, and support discharge planning.
