Job summary

Ignite Rehab, operating as part of Metro Physical & Aquatic Therapy, seeks a motivated per diem Home Care Physical Therapist to provide in-home services across Union County. Flexible schedule with part-time hours and a focus on patient-centered care.

We offer both 1099 and W-2 options, training for new graduates, and a supportive team. Travel to patients' homes is required within the designated geographic area, with mentorship available for documentation and compliance.

Qualifications

• Physical Therapist Program Graduate.
• Licensed Physical Therapist in New Jersey or eligible.
• Home health experience preferred but not required; mentorship provided for documentation and compliance.
• Ability to travel to patients' homes within Union County.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans.
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family.
• Plan, prepare and implement customized therapy programs to restore function and promote independence.
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques.
• Provide timely and accurate documentation for each visit.
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning.
• Deliver care in compliance with agency policies and professional practice standards.

• Compensation: USD 70 - USD 75 - hourly

• Physical Therapist Program Graduate
• Licensed Physical Therapist in the state of New Jerseyor eligible.
