Job summary

LifeSpan Therapy is seeking a Physical Therapist (PT) to help residents regain mobility, reduce pain, and improve overall function after injury, surgery, or illness. You will assess residents, develop personalized treatment plans, and implement therapeutic interventions in collaboration with families and the care team.

Qualifications

• Must be a licensed Physical Therapist (PT) with ACOTE-accredited schooling.
• Current Indiana state licensure to practice physical therapy.
• Valid Indiana Operator's license.
• Experience in resident care, assessment and treatment planning.
• Basic computer literacy for documentation and communication.

Responsibilities

• Assess resident PT needs and develop individualized treatment plans.
• Execute therapy interventions and coordinate with nursing staff.
• Monitor progress and document outcomes accurately.
• Educate residents and families on exercises and prevention.
• Occasionally travel to other LifeSpan locations for coverage.
