LifeSpan Therapy seeks a licensed Physical Therapist (PT) to help residents recover mobility, reduce pain, and improve function after injury or illness. You will assess conditions, create personalized plans, and implement therapies in collaboration with families and the care team.

The role emphasizes patient education, progress monitoring, and adapting treatments to achieve long-term health in a compassionate, family-oriented environment. PRN/part-time availability is expected.

Competenze

• Must be a licensed Physical Therapist (PT) who is a graduate of an ACOTE accredited school.
• Current state licensure to practice physical therapy in Indiana.
• Must have a valid Indiana Operator's license.
• Demonstrated experience in resident care, including assessment and treatment planning.

Mansioni

• Ensure physical therapy services facilitate resident outcomes and comply with MDS procedures.
• Assess residents for PT needs at admission or as indicated.
• Coordinate PT admissions/discharges with the Interdisciplinary Team and nursing staff.
• Develop and implement individualized treatment plans based on resident needs.
• Supervise PTAs when appropriate and communicate plans to the care team.
• Monitor progress and document treatment outcomes per healthcare regulations.
• Educate residents and families on injury prevention and therapy techniques.
• Utilize basic computer skills to manage records and scheduling.
• Occasionally travel to other locations to provide PT coverage if needed.
