Job summary

LifeSpan Therapy is seeking a licensed Physical Therapist (PT) to join Solarbron Terrace as a PRN, Monday through Friday. You will help residents recover mobility, reduce pain, and improve physical function after injury, surgery, or illness, with patient‑centered care and a collaborative approach.

You’ll assess residents, create individualized treatment plans, and coordinate with the interdisciplinary team.

Responsibilities

• Assures PT services lead to optimal resident outcomes and follows MDS procedures.
• Assesses residents for PT needs at admission or as indicated.
• Coordinates PT admissions/discharges with the care team.
• Develops and implements individualized treatment plans.
• Performs therapy or assigns to PTA; communicates plan to staff.
• Monitors progress and documents outcomes per regulations.
• Educates residents and families on injury prevention and health maintenance.
• Utilizes basic computer skills to manage records and schedules.
• Occasionally travels to other locations for coverage as needed.
