BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for full-time or per diem roles throughout Baltimore County, Maryland.

Per diem candidates must have current home health and Oasis experience.

Role Overview

As a home care Physical Therapist, you will be a key member of a multidisciplinary healthcare team, providing rehabilitative care and skilled nursing to enable clients to remain at home.

Responsibilities:

• Make home visits to clients in designated areas.
• Perform diagnostic tests and measurements related to mobility, strength, balance, and safety.
• Develop and implement individualized care plans, including therapeutic exercises and gait training.
• Assess and update care plans regularly, collaborating with nursing staff.
• Educate clients and families on rehabilitative activities to promote health and safety.
• Document observations and interventions accurately using electronic tablets.

Qualifications:

• Current Maryland Physical Therapist (PT) license.
• Graduation from an APTA-approved program or equivalent, with proof via transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; experience with EMR preferred.
