BAYADA Home Health Care - Physical Therapist (PT) - Full Time

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a full-time opportunity performing home health visits. The office is located in Baltimore, MD, serving adult and geriatric clients in the Nottingham area.

As a home care Physical Therapist, you will be a vital part of a multidisciplinary healthcare team providing rehabilitative care and skilled nursing, enabling clients to remain at home.

Responsibilities

1. Make home visits to clients in designated territories.
2. Perform diagnostic tests and measurements, including mobility, joint range, transfer status, stability, ambulation patterns, muscle strength and endurance, balance testing, and safety assessments.
3. Develop and implement individualized care plans with manual therapy, gait training, balance, and other interventions.
4. Assess and revise care plans regularly, collaborating with nursing staff as needed.
5. Educate clients and their families on rehabilitative activities to promote health, safety, and independence.
6. Document observations, interventions, and evaluations accurately using electronic medical records on a tablet.

Qualifications

1. Current MD Physical Therapist (PT) license.
2. Graduation from an American Physical Therapy Association-approved program.
3. Ability to work independently and manage time effectively.
4. Strong interpersonal, organizational, and problem-solving skills.
5. Proficiency with computers; experience with EMR systems preferred.
