BAYADA Home Health Care - Physical Therapist (PT) Opportunity

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for full-time, part-time, or per diem opportunities. The role involves performing home health visits for our York office, located in York, PA. This office services adult and geriatric clients across York County.

Compensation: $5,000 Sign-On Bonus for full-time positions!

As a home care Physical Therapist, you will be a vital part of a multidisciplinary healthcare team, providing rehabilitative care and skilled nursing to enable clients to remain at home.

Responsibilities:

1. Make home visits to clients in designated territories.
2. Perform diagnostic tests and measurements, including mobility, joint range, transfer status, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments.
3. Develop and implement individualized care plans, including manual therapies, gait training, and balance interventions.
4. Assess and update care plans regularly, collaborating with nursing staff as needed.
5. Educate clients and family members on rehabilitative activities to promote health, safety, and independence.
6. Document observations, interventions, and evaluations accurately using electronic devices.

Qualifications:

1. Current PA Physical Therapist (PT) license.
2. Graduation from an American Physical Therapy Association-approved program.
3. Ability to work independently and manage time effectively.
4. Strong interpersonal, organizational, and problem-solving skills.
5. Proficiency with computers; experience with electronic medical records (EMR) preferred.
