Hale Nani Rehabilitation and Nursing Center is seeking a licensed Physical Therapist to evaluate residents, develop treatment plans, and obtain physician approvals for services. The PT will treat residents per the treatment plan and participate in discharge planning.

Responsibilities include documenting evaluations and treatments, supervising PTAs and aides, and screening residents for potential benefits from PT interventions. Strong communication and problem-solving skills are essential.

Formación

• Licensed to practice Physical Therapy in the applicable state or actively pursuing licensure.
• Graduate of an approved Physical Therapist program.
• Active involvement in discharge planning.

We are looking for a Physical Therapist who is licensed in the practicing state or is a graduate of an approved Physical Therapist program and is actively working toward licensure.

Summary

The Physical Therapist evaluates residents, develops effective treatment plans, and obtains approval for service from the referring physician. Treats residents according to the physician's treatment plan. Participates in discharge planning. Records evaluation, treatment, and charges. Supervises Physical Therapist Assistants and Aides. Screens residents for potential benefit from PT intervention.

Skills And Abilities

• Excellent oral and written communication skills.
• Demonstrates effective problem-solving skills.
• Hold licensure as outlined by the Board of Physical Therapy in the employment state.
