Responsibilities include evaluations, treatment plan development, education for patients and families, collaboration with a multidisciplinary team, progress monitoring, and discharge planning.

Qualifications

  • Licensed Physical Therapist with a valid state license.
  • Experience in acute care, outpatient, or pediatrics is a plus.
  • Strong knowledge of discharge planning and medical terminology.
  • Excellent communication and team collaboration skills.

Responsibilities
